Adult Case Management (Intellectual Disability)
Individuals with intellectual disabilities are often challenged by a complicated service delivery system as they seek assistance in meeting their physical, social and emotional needs. Case managers assist individuals in determining eligibility for services and then work collaboratively with individuals and community partners to plan services which enhance the individual's quality of life. Throughout the stages of each person's life, the case manager monitors the delivery of services and advocates for consumers when needed.
Residential Services (Intellectual Disability)
Small congregate homes in Petersburg. Dinwiddie, and Emporia provide teaching and assistance to adults diagnosed with intellectual disabilities. Each home offers the support necessary to meet the physical, emotional and safety needs of each resident. Staffed 24 hours per day, individuals are presented with the opportunities to learn new skills and engage in activities that are important to them.
Residential Services (Mental Health)
Located in Dinwiddie County, the Ellis Square program offers transitional apartment living to adults diagnosed with a mental illness. Residents live in two bedroom apartments and receive supports necessary to teach them to live independently. The complex is staffed twenty four hours per day and offers a variety of leisure activities during the evening and weekend hours. Mental Health Supports are provided at this site.
Mental Health Support Services
Mental Health Support Services provide the teaching and assistance needed by many adults to maintain their independence. Provided individually, services may be take place in the individual's home or community and are available both weekend and evening hours. Consumers are supported as they develop the skills necessary to live independently.
