The Intensive Care Coordination (ICC) program provides the coordination of comprehensive community resources and services to children and adolescents who are at risk of entering or who are currently placed in a residential facility. The ICC services are short-term services provided to those children and adolescents who would benefit from a higher level of intensity than can be provided by typical case management services. Services and resources may be formal or informal and can include educational, financial, legal, vocational, medical, psychiatric, psychological, and social support opportunities. The ICC staff will work closely with District 19 Comprehensive Services Act (CSA) teams on a regular basis to provide intensive care coordination to CSA cases that are eligible for this service.
